Case Background: The Transformation from "Dissonance" to "Natural Harmony"

Client Xiao A (pseudonym) has long been troubled by the shape of her nose. Although her facial features have no obvious defects when viewed individually, her profile lacks three-dimensional definition. In frontal view, her nasal bridge appears slightly low and flat, and her nasal tip is rounded and blunt, causing the overall visual center of gravity of her face to shift downward and lacking refinement. During the consultation, she expressed a strong desire for improvement but also stated a clear core requirement: to reject the "internet celebrity fake look" and pursue a natural, harmonious appearance that looks innate.

Preoperative Assessment and Plan Design

After detailed palpation and imaging examinations, the doctor found that Xiao A's main problems were concentrated in the following three aspects:

Weak dorsal base: The starting point of the glabella was low, and the height of the nasal bridge was insufficient, resulting in an unsmooth side profile contour line.
Weak tip support: The alar cartilage development was average, the nasal tip projection points were unclear, and there was slight exposure of the nostrils.
Limited skin tension: The nasal skin was thin, and if the implant was too high, there was a risk of translucency or visible outline.

Based on the above assessment, the medical team formulated a "half-rib cartilage + expanded polytetrafluoroethylene (ePTFE)" comprehensive rhinoplasty plan. This plan aims to build a stable nasal tip scaffold using autologous cartilage while moderately elevating the nasal dorsum with materials having good biocompatibility, achieving a dual balance of structure and aesthetics.

Surgical Highlights and Technical Details

To ensure the durability and safety of postoperative results, the surgical process focused on refined operations:

Autologous rib cartilage harvesting: The 6th or 7th rib cartilage was selected as the material for nasal tip shaping. Autologous tissue has no rejection reaction, and its tough texture provides strong support for the nasal tip, effectively preventing postoperative nasal tip ptosis.
Fascia wrapping technique: Before implanting the nasal dorsum prosthesis, autologous fascia was used for multi-layer wrapping. This measure not only increased the thickness of soft tissue, buffered the pressure of the prosthesis on the skin, but also effectively reduced translucency, making the transition of the nasal dorsum lines more gentle and natural.
Nasal tip fine carving: The doctor adopted a combination of shield grafting and cap grafting to modify the nasal tip from multiple angles, lengthened the columella, reshaped clear nasal tip projection points, and adjusted the alar margin to improve the shape of the nostrils.

Postoperative Recovery and Effect Feedback

In the early postoperative period, the nose experienced normal swelling and bruising. After applying cold compresses and care according to medical advice, the symptoms subsided significantly within a week. At suture removal, the nasal shape had taken initial form, with smooth lines and no obvious scar hyperplasia.

During the three-month postoperative follow-up, Xiao A's nasal swelling had basically completely subsided, and the shape had stabilized. From the side, the connection between the glabella and the forehead was natural, and the nasal tip was slightly upturned without being abrupt; from the front, the nasal tip was small and exquisite, with proportions to other facial features more harmonious. She reported, "The current nose looks like it grew naturally on my face. Friends say I became prettier, but they can't specify what was changed. This is exactly the effect I wanted."

Doctor's Message

Rhinoplasty is not simply "elevating," but a systematic project involving aesthetic proportions, anatomical structures, and individual differences. A successful nasal correction should be based on respecting the individual's native foundation, using refined technical means to enhance the overall harmony of the face. It is recommended that clients communicate fully with doctors before surgery, set rational expectations, choose legitimate medical institutions and experienced physicians to ensure a win-win situation for medical safety and aesthetic effects.
