Project Background: Why Focus on "Mouth Corner Morphology"?

In facial aesthetics, the direction of the mouth corners often determines the "emotional tone" of the face. In a natural state, slightly upturned mouth corners convey a sense of approachability, gentleness, and vitality. Conversely, downturned or straight mouth corners can inadvertently reveal serious, fatigued, or even bitter expressions, even if the individual has no such intention internally. This "passive expression," caused by muscle traction or soft tissue laxity, is frequently a key concern for patients seeking to improve their youthful facial appearance.

Etiology Analysis: Common Factors Contributing to Downturned Mouth Corners

Downturned mouth corners are rarely caused by a single factor; they usually result from the combined effect of multiple elements:

Overactivity of the Depressor Anguli Oris Muscle: This is the most common cause. When the depressor anguli oris muscle, responsible for pulling the mouth corners down, exerts stronger force than the elevating muscles, the mouth corners tend to point downward even in a static state.
Soft Tissue Laxity and Gravity: With aging, the facial fascial layer loses tension, causing cheek soft tissues to descend and accumulate on both sides of the mouth corners. This forms "marionette lines" and pulls the mouth corners downward.
Skeletal and Dental Structure: Some patients naturally possess a skeletal foundation that causes the mouth corners to angle downward due to occlusal relationships or jawbone development characteristics.

Improvement Strategy: Core Logic of Minimally Invasive Adjustment

The core objective of minimally invasive treatments aimed at uplifting the mouth corners is to "balance muscle forces" and "support local volume," rather than simply forcing a lift. Doctors typically employ combination therapies to achieve natural, dynamically harmonious results:

Botox Relaxation of the Depressor Anguli Oris Muscle: By precisely injecting botulinum toxin, the contraction force of the depressor anguli oris is moderately inhibited, reducing its downward traction. This allows the elevator muscles to gain relative dominance, enabling a natural uplift of the mouth corners.
Hyaluronic Acid/Collagen Filling for Support: If accompanied by oral commissure bags or marionette line depressions, small amounts of filler can be injected lateral to and below the mouth corners. This provides physical support, reduces the downward pull of soft tissues on the mouth corners, and simultaneously fades wrinkles.

Case Study: Transformation from "Bitter" to "Approachable"

Pre-operative Status:
Patient A (pseudonym), 28 years old, reported that she appeared unhappy when not smiling, leading colleagues to mistakenly believe she was angry. Clinical examination revealed significantly downturned bilateral mouth corners, with the oral commissures positioned below the vermilion border level in a resting state, accompanied by mild perioral fine lines.

Treatment Plan:
Assessment indicated that Patient A's downturned mouth corners were primarily due to excessive tension in the depressor anguli oris muscles, with only mild soft tissue laxity. Therefore, a treatment plan focusing on Botox relaxation was formulated. Minimal injections were administered at the origin points of the bilateral depressor anguli oris muscles to balance perioral muscle dynamics.

Post-operative Feedback:
At the one-week follow-up, Patient A stated that her facial lines had become much softer in a resting state, no longer appearing serious. The arc of her smile was smoother and more natural, without stiffness. Notably, the minimally invasive adjustment preserved normal expressive function, improving the trajectory of the mouth corners only in a resting state to achieve an approachable enhancement described as a "subtle, gentle smile."

Professional Tips and Precautions

Although mouth corner uplift procedures fall under the category of light medical aesthetics, they demand high proficiency in anatomy and precise control of injection dosages. The following points require special attention:

Less is More with Dosage: Perioral muscles are delicate and complex. Excessive injection may lead to temporary functional issues such as asymmetric smiles, air leakage during speech, or difficulty eating.
Dynamic Assessment is Crucial: Muscle trajectories must be observed while the patient makes expressions (e.g., laughing, speaking) to avoid a "fake mask" appearance that looks good statically but appears stiff dynamically.
Reasonable Expectation Management: Minimally invasive treatments address muscle tension and mild volume loss. For severe skin laxity or skeletal issues, effects are limited, and comprehensive solutions involving energy-based devices or surgery may be necessary.
