Why Choose the Incision Method?

The patient reported that their baseline eye condition included sagging upper eyelid skin, thick fat accumulation (commonly known as "puffy eyes"), and mild levator muscle weakness. Although they had previously consulted about non-incisional (suture) double eyelid surgery, the doctor assessed that due to the thick eyelid tissue and redundant skin, the suture method would struggle to form a stable crease and was prone to losing its effect or resulting in poor shape because the thread could cut through the tissue. Therefore, after an in-person consultation and discussion, a comprehensive improvement plan consisting of "full incision double eyelid + skin and fat removal + levator muscle correction" was ultimately determined.

Surgical Plan Design

In response to the anatomical characteristics of the patient's eyes, the surgical design followed the principles of "natural appearance, symmetry, and functional improvement":

Incision Positioning: A double eyelid crease of moderate width was designed based on the distance between the eyebrows and eyes and the degree of eyeball exposure, avoiding the artificial deep-set look caused by overly wide creases.
Tissue Management: Excess loose skin and orbital septal fat were precisely removed to address upper eyelid puffiness, resulting in clearer eye contours.
Functional Adjustment: The aponeurosis of the levator palpebrae superioris muscle was appropriately folded or shortened to alleviate difficulty in opening the eyes, increase the exposure of the iris, and make the gaze brighter and more expressive.

Recovery Process Record

Post-operative Days 1-3 (Swelling Phase): Mild swelling and pain were present in the eyes, which is a normal post-operative reaction. Following medical advice, ice packs were applied to effectively constrict blood vessels, reduce exudation, and minimize swelling. The wound area was kept clean and dry, avoiding contact with water.

Post-operative Days 4-7 (Rapid Swelling Reduction Phase): Swelling gradually subsided, and bruising began to turn yellow and absorb. Hot compresses were applied daily on schedule to promote blood circulation. On the seventh day, stitches were removed at the clinic; the wounds healed well with no signs of obvious infection.

Post-operative 1-3 Months (Stabilization Phase): Incision marks gradually faded, turning from red to white. The double eyelid shape became increasingly natural, with only fine linear traces visible when closing the eyes. As the tissues softened, the smoothness of the double eyelid further improved.

Post-operative Effect Feedback

At the six-month post-operative follow-up, the patient expressed satisfaction with the results. The double eyelid lines were smooth and natural, with an appropriate width. This preserved individual characteristics while solving the previous issues of heavy eyelids and obscured pupils. When open, the eyes appeared lively and spirited; when closed, the scars were inconspicuous, achieving the expected aesthetic improvement goals.
